Yeast butt rash: how to respond

A possible yeast rash on the buttocks may be itchy, sore, red, shiny, or cracked, particularly where skin touches skin. Heat, sweat, tight clothing, and friction can keep the area damp. Contact dermatitis from laundry products, wipes, or topical medicines may cause a similar reaction. Atopic dermatitis, intertrigo, psoriasis, ringworm, scabies, and heat rash also belong in the differential. A clinician will consider the exact border, whether there are nearby small spots, what exposures occurred, and whether the rash is present elsewhere. A visual guess is not a diagnosis.

Use gentle cleansing and pat the area dry after washing. Change out of sweaty clothes, select breathable underwear, and avoid prolonged sitting in damp fabric. Stop newly introduced fragranced products and avoid scrubbing or scratching. Do not use multiple medicated creams at once, since this can irritate skin and make the pattern harder to interpret. For a persistent or recurring rash, ask a clinician or pharmacist which treatment is appropriate and whether testing is needed.

Contact a clinician promptly for fever, pus, spreading redness, increasing heat, marked pain, bleeding, or open sores. Seek emergency care for breathing difficulty, swelling of the lips or tongue, faintness, or widespread blistering. Arrange an appointment if the rash affects the anus or genitals, keeps returning, or is not settling with gentle care. If you have diabetes or reduced immunity, do not delay assessment.
