Why is ketoconazole used in medicated shampoo treatment?

Dandruff and seborrhoeic dermatitis can involve an interaction between scalp oil, skin inflammation and a yeast that normally lives on skin. Ketoconazole is an antifungal ingredient applied to the scalp to reduce that yeast and calm the cycle that contributes to scaling. It does not work like a cosmetic shampoo that simply loosens flakes.

Improvement may take repeated use, and the condition can return when treatment stops. The correct schedule depends on the product and the scalp problem. Redness, thick plaques, hair loss or a rash beyond the scalp may signal another diagnosis or a need for a different treatment.

Apply the shampoo to the scalp rather than only to the hair, and leave it on for the time stated by the clinician or product instructions before rinsing. Avoid the eyes and wash hands after use. Tell the clinician about scalp wounds, allergies and other products applied to the scalp.

Use it on the advised schedule, even if flakes improve quickly, and do not increase frequency to chase faster results. A gentle shampoo on other wash days may be more comfortable. Keep the bottle away from children and stop to seek advice if burning or a rash becomes pronounced.

Arrange a review if scaling or itch does not improve after the planned course, returns rapidly, or is accompanied by broken hairs, tender areas or patchy hair loss. Seek urgent help for facial swelling, breathing difficulty or a severe blistering reaction after application.

Ask whether your scalp changes fit dandruff, seborrhoeic dermatitis, psoriasis or another condition; how long to leave the shampoo on; and whether you need maintenance use. Clarify what to do if the scalp becomes irritated or symptoms spread.
