Why Does Vertebral Compression Fracture Cause Lower Back Pain?

A vertebral compression fracture occurs when one of the block-like bones in the spine loses height under load. The collapse can irritate the bone itself and the ligaments, joints, and muscles around it. That irritation may produce a sudden or gradually increasing ache in the lower back, often worse with standing, walking, bending, or changing position. Muscle tightening around the injured area can add stiffness and make movement feel restricted. Have new, significant, or persistent pain assessed, particularly after a fall, lift, or sudden movement. A clinician may use an examination and imaging to clarify the cause, then discuss pain control, activity changes, spinal support, and exercises suited to your condition.

Seek urgent medical help for new leg weakness, numbness around the groin, loss of bladder or bowel control, or difficulty walking. Prompt assessment is also appropriate after a fall or other injury when pain is severe or you cannot move normally. Contact a clinician soon if pain is worsening, wakes you repeatedly, or limits basic activities, because examination and imaging may be needed to assess the spine.

Ask which vertebra appears affected, whether imaging is needed, what activities are safe, and how to protect your back while it heals. Clarify which pain symptoms are expected, whether you need a brace or physiotherapy, and how bone health will be assessed if the fracture followed minor force.
