Why does hives cause skin rash?

Hives, or urticaria, begin when immune cells in the skin release mediators such as histamine. Small blood vessels widen and become more permeable, allowing fluid to collect just beneath the surface. This creates raised welts that may be pale, pink, or red and can join into larger areas. The borders may change shape as the swelling shifts.

A hive often fades from one place while another appears, unlike many rashes that remain fixed. Pressure, heat, infections, medicines, foods, or skin contact may contribute, although no trigger is found in some episodes. A flat, scaly, blistering, bruising, or painful rash needs assessment because it may not be hives.

Cool the skin with a compress, avoid hot showers, and choose loose clothing while the rash is active. Do not scratch or rub the welts, since friction can intensify local swelling. Ask a pharmacist or clinician whether an antihistamine is appropriate for you. If episodes repeat, note timing, recent medicines, foods, illness, heat, pressure, and new products so a clinician can review possible patterns.

Call emergency services for swelling of the tongue, lips, mouth, or throat, breathing difficulty, wheezing, faintness, or trouble swallowing. Seek medical advice for a rash that lasts, returns often, becomes painful, leaves bruising, or comes with fever or feeling unwell. These features may require a different evaluation.
