Why does tonsillitis cause sore throat?

The tonsils sit at the back of the throat. When they react to an infection, their tissue can become red and enlarged, while surrounding throat tissue may also feel irritated. Swallowing, talking, coughing, or eating rough foods can then rub or stretch sore tissue.

Most people with Tonsillitis notice sore throat. Pain may be stronger on one side or both, and it can travel toward the ears because the throat and ears share nerve pathways. Fever, tender glands in the neck, tiredness, a muffled voice, bad breath, and white or yellow coating on the tonsils can occur as well.

Tonsillitis can be caused by viruses or bacteria. The cause affects the assessment and whether specific treatment is appropriate, so symptoms alone cannot reliably identify the cause.

Rest, drink fluids regularly, and choose soft foods such as soup, yoghurt, or mashed foods if swallowing hurts. Cool drinks or ice lollies may soothe the throat for some people. Avoid smoke and other irritants, which can add to throat discomfort.

A pharmacist or clinician can advise on suitable pain relief and fever medicines, including options for children, pregnancy, allergies, or other health conditions. If antibiotics are prescribed, take them exactly as directed and do not save doses for a future illness.

Seek urgent medical care if breathing is difficult, you cannot swallow saliva, you are drooling, your voice changes markedly, or you cannot open your mouth normally. These signs can indicate significant swelling or a collection of infection near a tonsil.

Arrange a clinical assessment for worsening pain, dehydration, persistent fever, a rash, neck swelling, or symptoms that are not settling. Children and people with weakened immune systems may need earlier advice.

You could ask whether your examination suggests a viral or bacterial cause, whether a throat test is useful, which pain relief suits you, and what changes should prompt urgent review. If tonsillitis keeps returning, ask how the pattern should be documented and whether referral is appropriate.
