Why does tension headache cause neck stiffness?

Short answer
Tension headache can cause neck stiffness because tightened neck and scalp muscles create a pulling, aching sensation.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical mechanism
- Muscle tightening around the neck, shoulders and scalp
- Useful observation
- Posture, sleep, stress, meals and jaw clenching
What may be happening
Muscles at the back of the head, along the neck and across the shoulders can stay contracted when you are stressed, tired, sitting in one position, or straining your posture. The tension may be felt as a band-like headache, pressure around the head, soreness on touch, or limited comfort when turning the neck. Neck Stiffness is a common presentation of Tension Headache, but stiffness also has other causes.
A tension headache does not usually involve injury to the brain. The discomfort can still interfere with concentration, sleep and ordinary activities, especially if muscle tension keeps returning.
What you can do
Try a brief change of position, gentle neck and shoulder movement within comfort, and regular breaks from a screen or desk. A warm shower or heat pack may relax sore muscles; protect your skin from excessive heat. Drink fluids, eat regular meals and notice whether jaw clenching, poor sleep, stress or a workstation setup seems to precede episodes.
Simple pain relief may suit some people, but check the label and ask a pharmacist or clinician if you have a medical condition, take other medicines, are pregnant, or need it often. Frequent headache medicine use can itself contribute to recurring headaches.
When to seek care
Seek urgent medical assessment for a sudden severe headache, fever with a stiff neck, confusion, fainting, new weakness, trouble speaking, a seizure, or headache after a significant head injury. Arrange a clinical review if headaches are becoming more frequent, changing in pattern, waking you from sleep, or not improving with self-care. A clinician can examine your neck, nerves and headache pattern.
Questions to discuss
Ask whether your symptom pattern fits tension headache, which neck movements are suitable for you, and whether your medicines could be contributing. Bring a note of when the headaches occur, where the pain starts, associated symptoms, and what gives relief.

Questions people ask
Can a tension headache make it hard to turn my neck?
It can make movement uncomfortable because nearby muscles are sore or tight. Marked restriction, injury, fever or neurological symptoms need medical assessment.
Should I avoid moving my neck during a tension headache?
Avoid forceful movements, but gentle comfortable movement and changing position can be helpful for muscle tension.
