Why does stress urinary incontinence cause leaking urine?

The bladder stores urine, while the urethra carries it out. Muscles in the pelvic floor and the urethral sphincter help keep the outlet closed. During a sudden pressure change, the bladder is squeezed. If the supporting muscles, connective tissues, or sphincter do not counter that pressure quickly enough, a small amount of urine can leak. Stress here means physical strain, not emotional stress.

Pregnancy, childbirth, ageing, pelvic surgery, chronic coughing, and constipation can affect these support systems. Leakage may be occasional or may happen during several activities. Other bladder conditions can cause similar symptoms, so the pattern alone cannot establish a diagnosis.

Note when leakage occurs and whether urgency, burning, pelvic pain, or blood in the urine is also present. A clinician may ask about your bladder habits and may recommend pelvic floor muscle training, bladder strategies, treatment for constipation, or other options based on your assessment. Do not repeatedly stop urine midstream as an exercise; use separate pelvic floor contractions taught by a qualified professional.

Arrange an assessment if leakage is new, persistent, troublesome, or limiting exercise, work, intimacy, or sleep. Seek prompt medical advice for blood in the urine, fever, painful urination, severe back or pelvic pain, inability to pass urine, or sudden loss of bladder control with weakness or numbness.
