Why does shoulder impingement cause shoulder pain?

Short answer
Most people with Shoulder Impingement notice shoulder pain because irritated tendons and nearby tissue are compressed as the arm lifts.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical trigger
- Lifting the arm overhead
- Common tissues involved
- Rotator cuff tendons and bursa
What happens in the shoulder
The shoulder is a ball-and-socket joint with a narrow space beneath the acromion, the bony roof. When the arm rises, the rotator cuff tendons and bursa move through this space. Changes in movement, swelling, or reduced space can increase contact and irritation. Pain may be felt at the front or outer side of the shoulder and can be worse when reaching overhead, putting on clothes, or lying on that side.
Shoulder Pain is a very common presentation of Shoulder Impingement, but pain can also have other causes. A physical examination helps distinguish movement-related irritation from problems involving the neck, joint, or other shoulder tissues.
Helpful next steps
Temporarily reduce repeated overhead reaching and movements that clearly trigger pain, while keeping the shoulder gently moving within a comfortable range. A physiotherapist can assess shoulder mechanics and guide exercises for rotator cuff and shoulder-blade control. Use any pain relief or cold treatment only as directed for your situation; a clinician can help if symptoms persist or limit sleep and daily tasks.
When to seek care
Arrange an assessment if pain continues, progressively restricts movement, or follows an injury. Seek urgent care for sudden severe pain with inability to move the arm, a visibly deformed shoulder, new numbness or weakness, or chest discomfort with breathing trouble or sweating.
Questions for your doctor
Ask what examination findings support impingement, which movements to modify, and whether you need physiotherapy. You can also ask how to protect sleep, what improvement should look like, and when imaging or a different diagnosis should be considered.
