Why does prostate cancer cause persistent bone pain?

Prostate cancer cells may travel to bone and change the normal balance between bone breakdown and repair. This can weaken or irritate affected areas and activate pain-sensing nerves. The discomfort may feel deep, persistent, or worse at night, and it can occur in the back, hips, pelvis, ribs, or thighs. Affected bone may also be more vulnerable to injury.

Bone pain has many other causes, such as arthritis, muscle strain, osteoporosis, or an old injury. Persistent bone pain alone does not establish prostate cancer. If prostate cancer is known or suspected, a clinician may use an examination, blood tests, and imaging to assess the prostate, bones, and cause of pain.

Arrange a prompt medical review for new or ongoing deep bone pain, especially with urinary changes, fatigue, weakness, or a history of prostate cancer. Describe its location, timing, severity, and effect on movement or sleep. Keep taking prescribed medicines as directed and ask before using anti-inflammatory painkillers if you have kidney, stomach, or heart problems.

Seek emergency help for sudden severe back pain with leg weakness, numbness, difficulty walking, or loss of bladder or bowel control. These symptoms may indicate pressure on the spinal cord. Urgent assessment is also needed after a minor injury if a bone is suddenly very painful or difficult to use.

Ask whether imaging is needed, which pain-relief options fit your health, and whether treatment to protect bone strength should be considered. Ask what new weakness, numbness, or bladder symptoms should prompt emergency care.
