Why Does Plica Syndrome Cause Knee Clicking and Grinding?

Short answer
Plica syndrome causes knee clicking and grinding when an irritated fold of joint lining thickens, rubs against nearby structures, or briefly catches as the knee bends and straightens.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical mechanism
- An irritated plica rubs or catches as the knee moves.
- Common location
- The front or inner side of the knee.
- Useful next step
- A clinical assessment of movement, strength, and tenderness.
What the noise means
A plica is a thin fold of the synovial lining inside the knee. Repeated bending, kneeling, running, or another irritation can make it swollen and less flexible. It may then move over the inner side of the thighbone or along the edge of the kneecap, creating a snap, click, or rough grinding feeling. Knee Clicking and Grinding is a common presentation of Plica Syndrome. The sound may be felt more clearly than it is heard, and it can occur with soreness at the front or inner side of the knee. Noise alone does not show exactly which tissue is responsible, because several knee structures can make similar sounds.
What you can do
Reduce activities that repeatedly trigger the catching or rubbing for a short period, while keeping gentle, comfortable movement. Ice after activity may ease irritation. A clinician or physiotherapist can assess kneecap movement, thigh strength, hip control, and flexibility; a plan may include gradual strengthening and changes to movement technique. Do not force the knee through a painful click or repeatedly test it. If symptoms continue, an examination helps separate plica irritation from problems such as cartilage, tendon, or meniscus conditions.
When to seek care
Arrange a medical assessment if clicking is accompanied by persistent pain, swelling, repeated catching, loss of full movement, or a feeling that the knee gives way. Seek prompt care after a significant injury, especially if you cannot put weight on the leg, the knee looks deformed, or it becomes hot and very swollen. These features need assessment because the sound may not be coming from the plica alone.
Questions for your doctor or physiotherapist
Ask what movement is reproducing the click, whether the tenderness fits an irritated plica, and which findings would suggest another knee problem. You can also ask which exercises are suitable now, how to modify stairs or running, and what change should prompt a follow-up visit.

Questions people ask
Does every clicking knee have plica syndrome?
No. Clicking can come from several tissues, and the sound by itself cannot identify plica syndrome. Pain, tenderness, catching, and the movement that triggers the noise help guide an examination.
Can plica syndrome improve without surgery?
Many people are first treated with activity adjustment, symptom relief, and a gradual physiotherapy programme aimed at strength and movement control. A clinician can advise if symptoms do not settle or another cause needs investigation.
