What is happening

The pelvic floor is a group of muscles and connective tissues beneath the bladder, bowel, and reproductive organs. These muscles help keep the urethra closed while the bladder fills, then relax when you pass urine. Dysfunction may involve weakness, excessive tightness, or timing problems. A cough, laugh, lift, sneeze, or change in position can raise pressure inside the abdomen; if the muscles do not respond well, urine may leak. Sudden urgency can also be harder to hold when the muscles and bladder signals are poorly coordinated.

The pattern can vary. Some people leak with exertion, while others feel a strong urge first or have difficulty emptying fully. Leaking urine has several possible causes, so the symptom alone does not identify pelvic floor dysfunction.

What you can do

Track when leakage occurs, whether urgency comes first, how often you pass urine, and any constipation or pelvic discomfort. This record can help a clinician distinguish pressure-related leakage from urgency-related symptoms. A pelvic health physiotherapist can assess contraction and relaxation rather than assuming that more squeezing is appropriate. Treatment may include tailored exercises, breathing and pressure-management practice, bladder training, or relaxation work. Avoid repeatedly stopping your urine stream as an exercise, because it can interfere with normal emptying.

Arrange an assessment if leakage is new, persistent, worsening, or interfering with work, sleep, exercise, or intimacy. Seek prompt medical care for leakage with burning urine, blood in the urine, fever, new severe pelvic or back pain, leg weakness, numbness around the genitals, or loss of bowel control. These features need evaluation because infection, nerve problems, or another bladder condition may require different care.
