How a panic episode can feel in the chest

During a panic episode, the nervous system prepares the body for danger even when there is no physical threat nearby. Breathing may become rapid or shallow, the heart may beat harder, and muscles across the chest, shoulders, and ribs may tense. These changes can produce aching, stabbing, pressure, burning, or a tight sensation. Chest Pain is a very common presentation of Panic Disorder, but chest pain can also have physical causes that need medical assessment.

Fear of the sensation can add another layer: attention becomes fixed on the chest, and the feeling may seem stronger. This does not mean the distress is imagined; the physical sensations are real.

What to do during symptoms

If you have previously been assessed and recognise a panic pattern, move to a safe place, loosen restrictive clothing, and slow your breathing gently rather than taking large fast breaths. Try placing both feet on the floor and naming objects you can see, hear, and feel to bring attention back to the present. A clinician can discuss talking therapies, medicines when appropriate, and a personal plan for recurrent attacks.

Keep a brief record of episodes, including the first symptom, what was happening beforehand, how long symptoms lasted, and what helped. This can distinguish a recurring pattern from new symptoms.

When to seek emergency care

Seek emergency help for new, severe, crushing, or persistent chest pain; pain spreading to the arm, jaw, back, or neck; fainting; severe shortness of breath; a cold sweat; or symptoms after exertion. Seek urgent assessment when chest pain is different from your established pattern or you are unsure of its cause.
