Why does osteoarthritis cause joint pain?

Short answer
Osteoarthritis can cause joint pain when changes in cartilage, bone, and surrounding tissues irritate the joint during movement.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common pattern
- Pain and stiffness that can vary with activity and rest.
- Useful support
- Targeted movement and muscle strengthening can support the joint.
What happens inside the joint
Cartilage normally provides a smooth covering where bones meet. In osteoarthritis it can become thinner or uneven, so the joint no longer moves as smoothly. The bone beneath may change shape, and the joint lining, capsule, tendons, and muscles can also become irritated or strained. Pain may be worse after activity, after a period of rest, or when the joint is stiff.
Joint Pain is a very common presentation of Osteoarthritis. That relationship does not identify the cause of pain in any particular person: injuries, inflammatory conditions, and problems outside the joint can also cause similar symptoms.
What you can do
Keep the joint moving with comfortable, low-impact activity rather than stopping all movement. A clinician or physiotherapist can help tailor strengthening and flexibility work to the affected joint. Heat may ease stiffness for some people, while a wrapped cold pack may help after a flare. Discuss pain relief with a pharmacist or clinician, especially if you have stomach, kidney, heart, or liver concerns or take regular medicines.
When to seek care
Arrange a medical assessment if joint pain persists, limits walking or daily tasks, wakes you from sleep, or is accompanied by swelling. Seek urgent care for a hot, red, very swollen joint, fever, a new inability to bear weight, or pain after a significant injury. These features need assessment because they can have causes other than osteoarthritis.
Questions for your doctor
Ask which structures seem to be causing your pain, whether an examination or imaging would change management, and which exercises are suitable for your joint. It is also useful to ask how to pace activity during a flare and which pain medicines fit your medical history.

Questions people ask
Can osteoarthritis pain come and go?
Yes. Symptoms can fluctuate with activity, sleep, muscle fatigue, and periods of irritation around the joint.
Does joint pain always come from osteoarthritis?
No. A clinical assessment considers the location, timing, swelling, injury history, and other possible causes.
