Why does oesophageal cancer cause difficulty swallowing?

The oesophagus is the muscular tube that carries food from the mouth to the stomach. A cancer within its lining can take up space or stiffen the wall, so food may feel as though it sticks behind the breastbone. Some people change to softer foods, take longer to eat, cough during meals or bring food back up. Difficulty Swallowing is a very common presentation of Oesophageal Cancer, but swallowing difficulty has several possible causes and needs assessment rather than self-diagnosis.

Make a prompt medical appointment for swallowing difficulty that is new, persistent or getting worse. Describe whether the problem is with solids, liquids or both, where food seems to stick, and whether eating causes pain or coughing. Eat slowly, take small mouthfuls and choose textures you can swallow safely while waiting for advice. A clinician may arrange examination and tests to look at the oesophagus and identify the cause.

Seek urgent medical care if you cannot swallow liquids or saliva, feel food is completely stuck, have trouble breathing, or cough and choke repeatedly while eating. Also seek prompt assessment for vomiting blood, black stools, persistent chest pain, unintentional weight loss or worsening weakness. These signs require medical evaluation regardless of the underlying cause.

Ask what could be causing the swallowing problem, which test is needed to examine the oesophagus, and how to maintain hydration and nutrition safely. If a narrowing is found, ask what support is available for symptoms while the cause is being investigated.
