Why do myelodysplastic syndromes cause persistent fatigue?

Why energy can fall
Persistent Fatigue is a very common presentation of Myelodysplastic Syndromes. When the marrow produces red blood cells that are too few or do not work effectively, muscles and organs may receive less oxygen. This can cause weakness, breathlessness during activity, headaches or a racing heartbeat. Low platelets can lead to bleeding or bruising, while low white blood cells can make infections more concerning. Inflammation, poor sleep, reduced appetite and treatment effects may deepen tiredness.
Fatigue is not a measure of personal effort. Its pattern can change with blood counts, infection, bleeding, treatment and recovery, so describe changes rather than trying to push through them.
Managing day to day
Share the effect of fatigue on ordinary tasks with your haematology team. They may review a full blood count, bleeding or infection symptoms, medicines, nutrition and sleep, then discuss options suited to the cause. Keep a record of energy levels, dizziness, breathlessness, temperature, bleeding and treatment days. Use pacing: divide tasks, sit for activities when possible and schedule breaks before exhaustion arrives. Gentle movement may help preserve strength if your team considers it safe. Avoid supplements or over-the-counter medicines unless checked first.
Warning signs
Seek urgent medical advice for fever, chills, a new cough, worsening breathlessness, fainting, unusual bleeding, black stools, blood in urine, severe headache or rapidly increasing weakness. A low blood count or infection can require prompt treatment. Emergency help is needed for severe breathing difficulty, heavy bleeding, collapse, chest pain or sudden confusion.
