Why does multiple myeloma cause persistent bone pain?

Short answer
Multiple myeloma can cause persistent bone pain because abnormal plasma cells disturb bone remodelling, weaken bone, and irritate nearby tissues.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common mechanism
- Increased bone breakdown can leave fragile, painful areas.
- Common locations
- The spine, ribs, pelvis, and load-bearing bones may be affected.
- Urgent warning
- New weakness, numbness, or bladder or bowel changes needs urgent help.
How myeloma affects bone
Persistent Bone Pain is a very common presentation of Multiple Myeloma. Myeloma cells can release signals that increase bone breakdown while reducing the normal rebuilding response. This may create weakened areas, often in the spine, ribs, pelvis, or other load-bearing bones. Pain can feel deep and aching, remain in one area, or worsen with movement, coughing, or turning in bed.
Bone pain has other possible explanations, including injury, arthritis, infection, and osteoporosis. A clinical assessment may include questions about pain and movement, blood tests, and imaging. The pattern of pain cannot diagnose myeloma on its own, but a new or changing symptom deserves attention.
Managing the next step
Tell your haematology team where the pain is, whether it is new, and how it affects sleep or movement. Report falls, sudden pain, height change, weakness, tingling, or numbness. Protect a painful area from heavy lifting and unplanned impact until it is assessed. Take prescribed pain relief as directed and check before using anti-inflammatory medicines or supplements, particularly if you have kidney problems or other treatments.
When urgent help is needed
Seek urgent care for sudden severe back or limb pain, inability to move, new weakness, numbness, or loss of bladder or bowel control. These signs can occur with a fracture or pressure on the spinal cord. Contact the treating team promptly for pain that is escalating, pain after a small injury, repeated falls, confusion, severe thirst, or reduced urination.
Questions for your doctor
Could this pain indicate a weakened bone or fracture? Do I need imaging or blood tests? Which pain medicines are suitable for my kidneys and current treatment? Should I change lifting or exercise? What symptoms mean I should attend urgent care?
Questions people ask
Is persistent bone pain a very common presentation of multiple myeloma?
Persistent Bone Pain is a very common presentation of Multiple Myeloma, although other bone and joint conditions can cause similar pain.
Why can a small movement cause sudden pain?
A weakened area of bone may not tolerate ordinary loading, so a small movement can aggravate it or contribute to a fracture.
