Why can a missed miscarriage cause bleeding in early pregnancy?

In a missed miscarriage, the pregnancy has stopped developing but the body may not immediately expel the pregnancy tissue. As the womb lining changes and tissue starts to detach, blood can pass through the vagina. Bleeding may be light spotting, a heavier flow, or bleeding that comes and goes. Cramping, a change in pregnancy symptoms, or no symptoms at all can also occur. Early pregnancy bleeding has several possible causes, so bleeding alone cannot identify the cause.

Contact your maternity service or clinician for advice, especially if bleeding is new or becoming heavier. They may ask about the colour and amount of blood, pain, pregnancy dates, and whether you feel faint. Assessment can include an ultrasound and blood tests, depending on your situation. Use pads rather than tampons while bleeding so you can observe the flow, and keep a record of changes. Do not blame yourself; everyday activity does not cause a missed miscarriage.

Seek urgent medical help for heavy bleeding, severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, fever, or feeling very unwell. These signs need prompt assessment because early pregnancy bleeding can sometimes accompany an emergency, including ectopic pregnancy or significant blood loss.
