Why does lichen simplex chronicus cause thickened skin?

Lichen simplex chronicus often begins with an itch from irritation, eczema, an insect bite, dryness, or another trigger. Scratching briefly relieves the sensation but also injures the surface. The irritated area then becomes more sensitive, which encourages further rubbing.

Over time, the skin responds by becoming leathery, raised, and more visibly lined. The area may look darker or lighter than nearby skin and can feel rough rather than soft. Hair-bearing areas such as the neck, scalp, ankles, arms, or genital skin may be affected, but the diagnosis depends on the pattern and history rather than thickness alone.

Keep nails short, cover the area at night if scratching happens during sleep, and use a cool compress for sudden itch. Apply a plain moisturiser after washing and avoid heat, wool, fragrance, and rubbing when they worsen symptoms. A clinician may prescribe an anti-inflammatory treatment and address the original trigger; improvement depends on reducing scratching as well as calming inflammation.

Arrange an examination when thickening persists, spreads, cracks, oozes, bleeds, or interferes with sleep. Seek prompt care for severe pain, pus, fever, or a sore that does not heal. A clinician can check for infection, eczema, psoriasis, allergy, or another cause of itch when the appearance is not typical.
