Why does labyrinthitis cause vertigo?

What is happening in the inner ear

The labyrinth is part of the inner ear. It contains structures that help the brain judge head movement and body position. When inflammation affects this area, signals from the affected ear may no longer match those from the other ear or from your eyes and muscles. The mismatch can create a spinning sensation, unsteadiness, nausea, or difficulty focusing while moving.

Vertigo is a very common presentation of Labyrinthitis. Vertigo has other possible causes, so the symptom by itself does not identify the cause. Hearing changes, ringing in the ear, and imbalance can give a clinician useful clues when they assess you.

What you can do

During a spell, sit or lie down promptly and move your head slowly. Keep a clear route to the bathroom, avoid ladders and driving while you feel dizzy, and ask someone for help if you are unsteady. Small sips of fluid may be easier if nausea is present. Arrange a medical assessment, especially if the dizziness is new or interfering with normal activities.

When to seek urgent care

Seek urgent medical help for a new severe headache, fainting, chest pain, trouble speaking, facial weakness, weakness or numbness in an arm or leg, double vision, or inability to walk safely. These features need prompt assessment because they can have causes outside the inner ear. Also seek care if repeated vomiting prevents you from keeping fluids down.
