Why does IT band syndrome cause knee pain?

What is happening at the knee

The iliotibial band is a strong strip of connective tissue running along the outside of the thigh to the upper shin. As the knee bends and straightens, the band moves near the outer knee. A sudden increase in running, cycling, hill work, or other repetitive activity can overload this area. Irritation may cause a sharp or burning ache on the outside of the knee, often during activity and sometimes lingering afterwards. Tightness around the hip or thigh and reduced control of the hip and knee can add stress, but pain can also occur without an obvious single cause.

The discomfort is usually felt at one focused spot rather than throughout the knee. A clinician may compare both legs, assess movement, and consider other causes of outer-knee pain.

What may help

Reduce or pause the activity that brings on pain, especially hills, speed work, or long sessions. Keep gentle movement within a comfortable range so the knee does not become stiff. Ice wrapped in a cloth may calm symptoms after activity. A physiotherapist can guide gradual strengthening of the hip and thigh, movement retraining, and a stepwise return to exercise. Increase one training demand at a time and stop if pain becomes sharper or changes your walking pattern.

When to seek care

Arrange an assessment if pain keeps returning, limits walking, or does not settle after reducing the provoking activity. Seek urgent medical help if the knee becomes hot and markedly swollen, you develop fever, cannot bear weight after an injury, or the leg looks deformed. These features need evaluation because they can point to problems beyond activity-related IT band irritation.
