Why can an imperforate hymen cause delayed puberty?

The hymen is a thin tissue at the vaginal opening. With an imperforate hymen, it has no opening, so menstrual blood cannot pass out normally. Puberty-related breast development, body hair and other hormonal changes may still occur. The person may therefore appear to have delayed puberty because periods have not started, while the underlying hormone signals are progressing.

As fluid collects behind the tissue, cramping, pressure, lower abdominal or back pain, constipation, painful urination or a bulge at the opening may develop. A clinician uses the history and a gentle examination, and may use imaging, to distinguish this from hormonal causes of delayed puberty.

Book an appointment with a paediatric, adolescent or gynaecology clinician for an assessment. Keep track of breast development, body hair, pain, urinary symptoms and any bleeding. Do not try to open the tissue yourself or insert objects. If an imperforate hymen is confirmed, a minor procedure can create an opening and allow trapped fluid to drain; the clinician will explain timing, care and alternatives.

Seek urgent care for severe or rapidly increasing abdominal pain, vomiting, fever, inability to pass urine, fainting or a very swollen abdomen. Prompt assessment matters when menstrual fluid may be trapped, because pressure can affect nearby organs and pain can become intense.
