Why does hypothyroidism cause hoarse voice?

The vocal cords need to move freely and meet evenly to produce a clear sound. With an underactive thyroid, fluid can collect in tissues and the lining around the voice box can become puffy. The voice may sound lower, rougher, quieter, or tire more easily. Hoarse Voice is a common presentation of Hypothyroidism, although reflux, infection, voice strain, smoking, and other conditions can also alter a voice.

A change that develops gradually alongside tiredness, dry skin, constipation, feeling cold, or weight change is useful information to share at an appointment.

Book a routine assessment when hoarseness lasts, keeps returning, or affects speaking at work or home. A clinician may ask about voice use, reflux symptoms, medicines, and thyroid symptoms, and may arrange thyroid blood tests. Resting the voice, drinking fluids, and avoiding shouting may reduce strain while the cause is being assessed.

Do not assume the voice change comes from the thyroid, even if you have a previous diagnosis. Thyroid treatment should only be changed with clinical advice.

Get urgent medical help if hoarseness occurs with trouble breathing, noisy breathing, trouble swallowing saliva, swelling of the lips or tongue, or a rapidly worsening throat sensation. Seek prompt review for a persistent voice change, especially with coughing blood, a neck lump, or unexplained difficulty swallowing.

Ask whether your symptoms warrant thyroid testing, whether reflux or voice strain could be contributing, and when an examination of the throat or voice box is needed. Bring a list of medicines and mention how long the change has been present.
