Why does high calcium in cancer cause persistent fatigue?

Calcium helps cells communicate, muscles contract, and nerves work. When calcium in the blood becomes too high, these signals can become less efficient. A person may feel weak, sleepy, slowed down, or unable to concentrate. The kidneys may also produce more urine, which can lead to dehydration and worsen tiredness.

High calcium in cancer can develop through effects of cancer on bone or through chemical signals that change calcium handling. Persistent fatigue has many other causes, including anaemia, infection, pain, poor sleep, and treatment effects. A blood test is needed to assess calcium rather than relying on fatigue alone.

Tell your oncology team about new or worsening fatigue, increased thirst, frequent urination, constipation, nausea, weakness, or confusion. Keep track of fluid intake, alertness, bowel changes, and medicines, but do not change prescribed treatment without advice.

Your clinician may review medicines, hydration, kidney function, and blood calcium. Management depends on the cause and severity and may include supervised fluids or medicines that lower calcium. Follow the plan given to you, particularly if you have heart or kidney problems.

Contact your oncology team promptly for marked fatigue with vomiting, worsening weakness, very frequent urination, constipation that does not improve, or increasing thirst. Seek urgent help for confusion, difficulty staying awake, fainting, severe dehydration, or a seizure.

Ask whether calcium has been checked, what may be causing the change, how much fluid is suitable for you, which medicines could affect calcium, and which warning signs require urgent care.
