Why endometriosis causes pelvic pain

Endometriosis causes pelvic pain when endometriosis-like tissue triggers inflammation, irritation and scar tissue in the pelvis, often worsening around menstruation. Pelvic Pain is a very common presentation of Endometriosis, but the intensity and timing can vary from person to person.

What the pain can involve

Endometriosis-like tissue responds to hormonal changes during the menstrual cycle. Swelling and bleeding in places outside the uterus can irritate nearby nerves and organs, causing cramping or aching in the lower abdomen and pelvis. Repeated inflammation may lead to adhesions, which are bands of scar tissue that can make pelvic structures stick together or pull when they move. Pain may occur during periods, between periods, during sex, with bowel movements or when passing urine. Some people also experience lower back pain, bloating or pain that spreads beyond the pelvis. Pain severity does not always match the amount of tissue present, so a clinician may need to consider symptoms, examination findings and other tests together.

What you can do

Keep a symptom diary that records pain location, timing, menstrual cycle changes, bleeding, bowel or bladder symptoms and activities that make pain worse. This can help a healthcare professional identify patterns. Treatment may include pain relief, hormonal treatment, physiotherapy aimed at the pelvic floor, or surgery in selected situations. The right option depends on your symptoms, health history and plans for pregnancy. Do not start or stop prescription treatment without medical guidance.

When to seek care

Arrange a medical appointment if pelvic pain keeps returning, affects work or sleep, makes sex or daily activities difficult, or comes with heavy or irregular bleeding. Seek urgent care for sudden severe pain, fainting, marked weakness, fever, repeated vomiting, or heavy bleeding with dizziness. If pregnancy is possible and pelvic pain is severe or associated with bleeding, seek urgent assessment.

Questions for your doctor

You could ask: What other causes of my pelvic pain should we check? Could my bowel, bladder or pelvic floor be involved? Which treatment is suitable with my current medicines and pregnancy plans? What symptoms should prompt urgent review? Would pelvic physiotherapy or a specialist referral be useful?

Questions people ask

Is pelvic pain a common symptom of endometriosis?

Yes. Most people with Endometriosis notice pelvic pain, although its timing, location and severity differ. Pelvic pain can also have causes unrelated to endometriosis.

Why can endometriosis pain continue outside a period?

Ongoing inflammation, irritated nerves or adhesions may continue to affect pelvic tissues between periods. Recurrent or disruptive pain deserves a clinical assessment.
