Bring a family member or trusted friend if the person agrees, as they may be able to describe changes over time. Keep a brief record of examples, including when they happen and any medicines or illnesses around the same time.

Simple supports can reduce frustration: keep keys and medicines in consistent places, use a visible calendar, write short reminders, and give one instruction at a time. Speak calmly, allow time for an answer, and avoid testing or correcting the person repeatedly.

When to seek medical care

Seek urgent medical help if confusion appears suddenly, fluctuates over hours, follows a head injury, or occurs with fever, new weakness, trouble speaking, severe headache, or marked drowsiness. A sudden change is different from a gradual pattern and needs prompt assessment. For a gradual decline, book a clinical appointment rather than waiting for safety problems to develop.
