Why does complex regional pain syndrome cause hand swelling?

Hand Swelling is a common presentation of Complex Regional Pain Syndrome. After an injury, operation or other trigger, pain-processing nerves may become overactive. They can influence nearby blood vessels and the small vessels that control fluid exchange. More fluid may then collect in the hand, while circulation and drainage work less efficiently.

The hand may feel warm or cool, look different in colour, sweat more or less than the other side, and become stiff. Swelling can make finger movement uncomfortable, but keeping the fingers still for long periods may increase stiffness. These changes can fluctuate rather than follow a steady pattern.

Follow the movement and desensitisation plan given by your clinician. Gentle, comfortable finger and wrist movement can help preserve flexibility; do not force a painful stretch. Elevate the hand when resting if this is comfortable, and avoid tight rings, sleeves or straps while swelling is present.

Protect the hand from burns, cold injury and accidental knocks because altered sensation may reduce warning signals. Note when swelling changes, which activities affect it, and whether colour or temperature also changes. This record can help your physiotherapist or doctor adjust care.

Arrange a clinical review if swelling is new, increasing, limiting finger movement or accompanied by worsening pain or skin changes. Seek urgent help for sudden severe swelling with marked colour change, a cold or pale hand, new weakness, fever, spreading redness, chest pain or breathlessness. These features need assessment because other problems can affect the hand.
