Why does compartment syndrome cause shin pain?

Why the shin can hurt
The front of the lower leg contains muscles, nerves and blood vessels enclosed by firm connective tissue. Swelling or bleeding inside that space can raise pressure because the boundaries do not stretch easily. The resulting tension can cause deep shin pain, tightness or a sensation of fullness. Pain may worsen when the ankle or toes are moved because that stretches the affected muscles. Nerves under pressure can cause pins and needles, reduced feeling or weakness in the foot. This pattern can follow a fracture, crush injury, tight dressing or heavy swelling, and it needs to be distinguished from less urgent causes of shin discomfort.
What to do
Treat rapidly worsening shin pain after an injury as an emergency rather than exercising through it. Avoid massage and do not try to cut, loosen or modify a cast without medical advice. Tell emergency staff when the pain began, whether it is increasing, and whether movement, numbness or weakness has changed. They will examine the leg and may measure pressure within the compartment. If the pressure threatens muscle or nerve tissue, urgent surgery may be required to open the tight covering and relieve it.
When to seek emergency care
Seek immediate emergency care for severe or escalating shin pain, a firm swollen lower leg, pain when the foot or toes are gently moved, new tingling, numbness or weakness, or a foot that looks pale or feels cold. These signs can indicate worsening pressure. Do not wait for every sign to appear or rely on the visible appearance of the skin before seeking help.
