Why does bowel cancer cause persistent fatigue?

A tumour can cause slow blood loss from the bowel. Over time, this may contribute to iron deficiency or anaemia, which can leave you weak, breathless or light-headed because less oxygen reaches tissues. The body may also use more energy during an inflammatory response to illness. Pain, disrupted sleep, reduced appetite and the emotional strain of diagnosis can add to the exhaustion.

Fatigue is not the same as ordinary sleepiness: rest may not restore your usual capacity, and simple tasks can feel harder. Persistent fatigue has many possible causes, including anaemia from reasons other than cancer, thyroid problems, infection, medication effects and low mood.

Tell your clinician how fatigue affects walking, work, concentration, sleep and daily tasks. Mention breathlessness, dizziness, paleness, bleeding, appetite changes and bowel symptoms. A clinician may check for anaemia and other reversible contributors. Pace activities, plan rest between tasks, keep fluids and nourishing foods within reach, and ask for help with demanding jobs while the cause is being assessed.

Seek urgent medical help if fatigue comes with chest pain, severe breathlessness, fainting, confusion, marked weakness or heavy bleeding. Contact your care team promptly for worsening dizziness, black stools, visible blood or a sudden drop in your ability to function. These symptoms may need quick assessment for anaemia, bleeding or another urgent problem.

Ask whether anaemia or another condition could be contributing, which tests may clarify the cause, how to conserve energy safely, and which changes in breathing, dizziness or bleeding should trigger urgent help.
