Why does cauda equina syndrome cause bladder or bowel changes with back pain?

The cauda equina is a bundle of nerves at the lower end of the spinal canal. These nerves carry messages to and from the legs, pelvic muscles, bladder, and bowel. Compression can cause back or leg pain, altered sensation around the inner thighs or buttocks, difficulty starting urination, reduced awareness of bladder filling, or changes in bowel control. The pattern can develop with or without leg weakness.

New bladder or bowel changes alongside back pain should be treated as an emergency symptom pattern. Do not wait for a routine appointment or rely on home exercises. Seek emergency medical assessment immediately and describe when the change began, whether you can pass urine, and whether there is numbness around the saddle area.

Go to an emergency department immediately for difficulty passing urine, new loss of bladder or bowel control, numbness around the genitals or buttocks, or worsening weakness in either leg with back pain. Rapid assessment is needed because pressure on these nerves can threaten bladder, bowel, sexual, and leg function.

Tell the emergency team exactly what changed and when. Ask which neurological findings are being checked, whether urgent imaging is needed, and what symptoms should be reported while you are waiting.
