Why do bone metastases cause persistent bone pain?

Bone is constantly remodelled: one group of cells removes old bone while another builds new bone. Metastatic deposits can disturb this balance. Some areas become too thin and weak, while others become unusually dense but less flexible. Both patterns can create mechanical stress and pain.

The outer surface of bone contains pain-sensitive nerves. Expansion or inflammation beneath this surface can create a deep ache that persists at rest or worsens at night. A deposit near the spine, hip, or another load-bearing area may also affect movement and increase fracture risk. Pain alone cannot show whether a metastasis is present; assessment is needed.

Tell the oncology team about new pain, pain that is becoming stronger, or pain that changes with movement. Describe its exact location, timing, severity in daily life, and any weakness or numbness. The team may use examination and imaging to assess the bone and decide whether treatment should target the deposit, the underlying cancer, pain signals, or bone strength.

Use only the pain plan provided for you and ask before changing doses or adding medicines. Protect a painful limb or back from heavy loading until a clinician advises what is safe. Gentle movement may help some people, but an activity that causes sharp or escalating pain should stop.

Seek urgent assessment for sudden severe pain, a new inability to walk or use a limb, or a limb or back that looks deformed. These may indicate a fracture or significant weakening.

Spinal pain with new weakness, numbness, difficulty walking, or loss of bladder or bowel control is an emergency because pressure on the spinal cord can worsen quickly. Seek urgent advice as well for rapidly increasing swelling, fever, or severe pain that is not controlled by the prescribed plan.

Ask which bone is affected, whether it is structurally weak, and whether imaging is needed now. You can ask how treatment may protect the bone, which movements are safe, how to adjust pain relief, and which warning signs should prompt emergency care.
