Why does atopic dermatitis cause thickened skin?

During an atopic dermatitis flare, inflammation causes itching. Repeated scratching or rubbing creates small, ongoing injuries and encourages the outer layer of skin to build up. The normal skin lines can become deeper and more obvious, a change called lichenification. The area may feel rough, firm, dry, or leathery and can look darker or lighter than nearby skin.

Thickening commonly appears where a person can reach easily, such as the neck, wrists, ankles, hands, or skin behind the knees. It reflects a cycle of itch, rubbing, and barrier damage; it is not a sign that the skin has become healthier or stronger.

Moisturise with a fragrance-free product and use it consistently, including after bathing. Keep nails short, press or cool an itchy area instead of scratching, and reduce friction from rough clothing. Treating the inflammation early can help prevent further rubbing. A clinician may prescribe medication for a flare and explain how long to use it on thickened areas.

Seek an appointment if thickened patches are spreading, painful, bleeding, interfering with movement, or not softening as the itch improves. Prompt assessment is needed for pus, yellow crusting, marked warmth, swelling, fever, or rapidly worsening pain. A new firm patch that does not itch or changes independently should also be examined.
