Changes inside the airways

An allergic reaction can release chemicals that tighten the muscles around the smaller breathing tubes, increase mucus, and make airway tissues swell. A person may feel unable to draw a full breath, breathe faster, cough, wheeze, or notice a tight throat or altered voice. Shortness of Breath is a very common presentation of Anaphylaxis.

Breathlessness also has many non-allergic causes. In a possible allergic reaction, the timing after exposure and the presence of skin, stomach, circulation, or throat symptoms help clinicians assess the situation, but urgent care should not be delayed for assessment.

Immediate action

Use a prescribed adrenaline auto-injector immediately if anaphylaxis is suspected, then call emergency services. Remain with the person and monitor their breathing and responsiveness. Lie flat with legs raised when tolerated; a person struggling to breathe may sit with legs extended. Do not allow sudden standing or walking.

Inhalers or antihistamines do not replace adrenaline for anaphylaxis. Emergency clinicians can support breathing and decide whether further treatment is needed.

Get emergency help straight away for breathlessness after a possible allergen, noisy breathing, wheeze, throat tightness, swelling of the tongue, difficulty speaking, bluish or grey lips, fainting, or increasing drowsiness. Seek emergency assessment after adrenaline even if breathing improves, as symptoms can return.
