White watery discharge after menopause: what to know

After menopause, lower oestrogen levels can make the vaginal lining thinner and less able to maintain moisture. This may cause dryness, delicate tissue, or a small amount of pale discharge. A change in the usual vaginal bacteria can also cause discharge, itching, soreness, or an unpleasant smell. Thrush and other infections are possible as well, and symptoms alone cannot identify which cause is present.

Fluid can sometimes come from urine rather than the vagina, particularly when there is leakage with coughing, urgency, or a feeling of incomplete bladder emptying. A bulge, pressure, or difficulty emptying the bladder may point to pelvic organ prolapse. These symptoms need a clinical examination rather than self-diagnosis.

Notice when the discharge began, whether it is continuous, and any changes in colour, smell, or amount. Also record itching, burning, pain during sex, urinary leakage, or new products that may have irritated the area.

Wash gently with lukewarm water or an unscented cleanser on the outside only. Avoid douching, perfumed washes, and inserting over-the-counter treatments unless a clinician has confirmed the cause. A clinician may examine the vagina and cervix and take a swab or urine sample when infection or urine leakage is suspected. Treatment depends on the finding and may include an appropriate vaginal moisturiser, local oestrogen, or medicine for an infection.

Arrange a medical appointment for discharge that is new, persistent, recurrent, or different from your usual pattern. Seek prompt assessment for any vaginal bleeding after menopause, pink or brown fluid, a strong or foul smell, pelvic or lower-abdominal pain, fever, sores, or marked itching and burning. Urgent care is appropriate for severe pain, heavy bleeding, faintness, or feeling acutely unwell.

Do not wait for the discharge to become painful before arranging an assessment. Bleeding after menopause has several possible causes, many of which are treatable, but it should always be checked.
