When to worry about swollen ankles with heart failure

Swollen Ankles is a very common presentation of Heart Failure. When the heart is not moving blood effectively, pressure can build in the veins and fluid may collect around the ankles and lower legs. New swelling or a clear increase from your usual pattern can be a sign that fluid balance needs review.

Compare the swelling with your usual baseline and record changes in breathing, cough, activity, sleep, and urine. Take prescribed medicines as directed and follow the fluid and salt advice from your care team. Do not double a diuretic dose unless your clinician has given you a specific plan. Rest with your legs supported if comfortable and call for advice about a clear change.

Call emergency services for severe or sudden breathlessness, chest pain, fainting, blue lips, confusion, or inability to speak comfortably because of breathing. Seek same-day medical advice for swelling that is rapidly increasing, breathlessness at rest or when lying down, a worsening cough, or a noticeable drop in exercise tolerance.
