When to worry about slurred speech with hypoglycaemia

Low blood glucose deprives the brain of its usual fuel. Speech may become unclear, and a person may also feel shaky, sweaty, hungry, irritable, confused, weak, or unusually sleepy. These changes can develop quickly. Slurred speech can also have other causes, so do not assume every episode is hypoglycaemia, especially if the glucose reading is not low or the symptom does not improve after treatment.

If the person is awake, able to swallow safely, and a glucose check is available, check it and follow their agreed low-glucose plan. Give a fast-acting source of sugar recommended for them, then recheck or reassess according to that plan. Stay with them and avoid driving. Do not give food, drink, or tablets to someone who is drowsy, having a seizure, unconscious, or unable to swallow; place them safely on their side and call emergency services.

Call emergency services if slurred speech is severe, sudden, worsening, or accompanied by confusion, collapse, a seizure, weakness on one side, facial drooping, breathing difficulty, or inability to swallow. Seek urgent medical advice when symptoms do not settle after the person’s usual treatment, keep returning, or occur without a clear low-glucose reading. A clinician can review medicines, meals, exercise, alcohol use, and monitoring technique.

Ask whether your medicines, meal timing, exercise, or alcohol could be contributing, and whether someone close to you should learn how to respond if you cannot swallow.
