What causes knee clicking and grinding?
A noise by itself does not identify one diagnosis. The useful clues are whether it started after an injury, whether it causes pain or swelling, and whether the knee catches, locks, or gives way.
What the sound may mean
A painless pop may occur when a tendon or ligament moves over nearby bone, or when pressure changes inside the joint release a small gas bubble. Crunching behind or around the kneecap can occur when the kneecap and thighbone do not glide smoothly during bending, stairs, or squats; chondromalacia patellae is one condition associated with this pattern.
Rough grinding with stiffness or reduced movement can accompany knee osteoarthritis, where joint surfaces and surrounding tissues change over time. A click after twisting, especially with swelling, catching, or difficulty straightening, can point toward a meniscus tear. A plica syndrome can also cause a snapping sensation along the inner side of the knee.
What you can do now
If the sound is painless and your knee works normally, notice when it happens and avoid repeatedly forcing the movement to test it. Keep gently active, choose activities that do not provoke pain, and build strength around the hip and thigh gradually. After a recent strain, relative rest and a wrapped cold pack may help with irritation.
Arrange an assessment if the noise persists with pain, swelling, stiffness, weakness, or reduced function. A clinician or physiotherapist can examine kneecap movement, joint range, muscle control, and signs of meniscus or ligament injury; scans are not needed for every noisy knee.
When to seek care
Seek prompt medical care after a significant injury if the knee is visibly deformed, rapidly swollen, or difficult to bear weight on. Arrange urgent assessment when the knee is locked and cannot fully bend or straighten, or when it repeatedly gives way.
Contact a clinician soon for a hot, red, markedly swollen knee, fever with knee symptoms, or pain that is worsening rather than settling. These features need evaluation because the cause may involve more than ordinary joint noise.

Is knee clicking without pain serious?
