What causes chronic ankle instability?

Short answer
Chronic ankle instability is most often caused by an incompletely recovered ankle sprain that leaves the outer ankle support or joint control impaired.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical starting point
- A previous ankle sprain
- What may remain affected
- Ligament support, movement, strength, or balance control
- Common complaint
- The ankle gives way on uneven ground or during turning
Why the ankle keeps giving way
An ankle sprain can stretch or tear the ligaments on the outside of the joint. If the tissue remains loose or the ankle does not regain its full movement, strength, and balance responses, the foot may roll again during uneven walking, turning, or sport.
Repeated sprains can further irritate the joint and reinforce an unstable movement pattern. Less common contributors include a fracture that did not heal as expected, tendon problems, poor foot control, or an underlying alignment issue. A giving-way history helps the clinician decide what needs checking.
What to do next
Assessment may include ankle movement, ligament tenderness, calf and foot strength, single-leg balance, walking, and sport-specific tasks. Rehabilitation commonly rebuilds range of motion, calf and foot control, balance reactions, and confidence before demanding activity is resumed. A brace or tape may provide short-term support during higher-risk activities.
When to seek care
Arrange an assessment if the ankle repeatedly gives way, remains swollen, or cannot tolerate normal walking after a sprain. Seek urgent care after a new injury if you cannot bear weight, the ankle looks deformed, pain is severe, or the foot becomes numb, pale, or unusually cold.
Questions for your doctor
Ask which ligament or structure appears affected, whether a fracture or tendon problem needs excluding, and which balance and strengthening exercises fit your stage of recovery. Ask when a brace is useful and how return to sport will be tested.

Questions people ask
Can one ankle sprain cause chronic instability?
Yes. A single sprain can leave the ligaments or the ankle's movement and balance control incompletely recovered.
Can chronic ankle instability improve with physiotherapy?
Many people are assessed for a rehabilitation plan that targets movement, strength, balance, and activity control.
