Weight gain and bleeding between periods

Weight gain with bleeding between periods can have separate or related causes, so unexpected bleeding should be assessed rather than explained by weight alone. Changes in ovulation, contraception, pregnancy, thyroid function, medicines or the womb lining can affect bleeding. Weight change may also reflect lifestyle, hormones or fluid, but it does not show which condition is present.

Bleeding between periods means bleeding outside your expected menstrual flow, including spotting after sex. It can occur with hormonal contraception, infection such as chlamydia, cervical dysplasia, polyps, fibroids or changes to the womb lining. A cervical or womb condition may present with intermenstrual bleeding, but the symptom does not diagnose that condition. Weight gain can sometimes accompany hormonal or thyroid changes, yet the two symptoms may be unrelated.

Record when bleeding starts, how long it lasts, whether it follows sex, and any pain, discharge or dizziness. Note period dates, contraception, medicines, pregnancy possibility, bowel or bladder pressure and weight trends. Arrange a gynaecology or primary-care review, particularly if bleeding is new, recurrent or occurs after menopause. Examination, pregnancy testing, infection testing or imaging may be considered according to your history.

Seek urgent care for bleeding with severe pelvic pain, fainting, marked weakness, shoulder-tip pain, or a possible pregnancy. Prompt review is also needed for bleeding after sex, bleeding after menopause, fever, unpleasant discharge or bleeding that is becoming heavier. These warning signs are driven by the bleeding pattern and accompanying symptoms, not by how much weight has changed.

Ask what may explain the bleeding; whether pregnancy, infection, thyroid function or medicines should be checked; whether cervical screening is due; whether an ultrasound or other test is appropriate; and which changes should lead you to seek urgent care.
