Vulvar Itching and Burning: What to Know

The vulva is the external genital skin, and symptoms there may be triggered by fragranced products, sweat, friction, shaving, tight clothing, or a change in the skin barrier. Burning can also occur with soreness, redness, scaling, cracks, swelling, or altered discharge. The location and appearance of the skin help a clinician distinguish among possible causes.

Contact dermatitis is irritation or an allergic reaction to something touching the skin, such as soap, detergent, wipes, pads, or topical products. Eczema can cause dry, itchy, inflamed skin, while psoriasis may produce sharply defined irritated patches. Lichen planus can affect delicate genital skin and may cause soreness, raw areas, or changes in texture. A yeast infection can cause intense itch, redness, and discomfort, sometimes with discharge. Scabies is another possible cause when itching is severe at night or other household members are itchy.

These symptoms do not identify one diagnosis by themselves. A clinician may ask about recent products, sexual health, periods, medicines, skin conditions elsewhere, and whether the discomfort is external or internal. Examination and testing may be needed when infection is possible.

Rinse gently with lukewarm water, pat dry, and pause fragranced washes, deodorants, wipes, douches, and new creams on the area. Wear loose, breathable underwear and change out of damp clothing. Avoid scratching, shaving irritated skin, and sexual contact if it worsens pain or causes skin breaks. Do not use leftover antibiotics, antifungals, or steroid creams unless a clinician has advised that product for this episode; the wrong treatment can irritate skin or mask the cause.

Arrange an appointment if symptoms persist, recur, or are accompanied by discharge, odor, sores, blisters, bleeding, or pain when passing urine. A clinician can choose treatment based on the skin findings and any test results.

Seek prompt medical care for rapidly increasing swelling, severe pain, spreading redness, pus, fever, new blisters or open sores, or difficulty passing urine. Get assessed soon for persistent skin color or texture changes, bleeding not explained by a period, or a sore that does not heal. Pregnancy, diabetes, immune suppression, or recent antibiotic use can change the evaluation and should be mentioned when booking.

Urgent assessment is especially appropriate when symptoms are severe, started after a new medicine, or occur with facial swelling or trouble breathing. Those features can signal a reaction that needs immediate attention.

Ask: Which areas of skin look affected, and what causes are you considering? Do I need a swab, urine test, or other examination? Which products should I stop, and what can I safely use for comfort? Could a medicine, allergy, eczema, psoriasis, or infection be contributing? What signs should prompt earlier review, and how should I prevent another episode?
