What should you know about vesiculobullous lichen planus?

Lichen planus can produce raised, itchy bumps and inflamed patches. In a vesiculobullous presentation, the skin may form tense or fragile blisters over those areas. When blisters rupture, they can leave erosions, crusts, and soreness. The same pattern may be confused with infection, a medicine reaction, burns, or autoimmune blistering, so a clinician may need a detailed examination and testing.

Protect blisters from friction, wash gently, and cover broken areas with a clean non-stick dressing. Avoid scratching and unapproved creams. A dermatologist may prescribe topical corticosteroids for limited areas. Oral corticosteroid treatment or another systemic approach may be discussed when the eruption is widespread, painful, or affecting sensitive sites. Follow the prescribed schedule and report new symptoms rather than changing the dose yourself.

Arrange prompt assessment for new widespread blistering, rapidly worsening pain, fever, pus, dehydration, or raw skin. Eye, mouth, genital, or breathing symptoms need urgent attention. Blistering that begins after a new medicine should be reviewed quickly, even if the skin area seems limited.

Ask whether a biopsy or blister-fluid test is appropriate, how to protect open areas, which body sites need special monitoring, and when treatment should be reassessed.
