What may be happening
You may become aware of vertigo while turning over, lying back, or sitting up rather than while fully asleep. Keep the path to the bathroom clear and consider asking someone to stay nearby if you feel unsafe. Avoid driving until the cause is clearer. Write down the sleeping position, movement that started the episode, duration, and associated symptoms. A clinician can determine whether an Epley Manoeuvre is suitable; do not attempt it if you have not been assessed or have neck, back, or circulation concerns.
When to seek urgent care
Call emergency services for vertigo with facial drooping, one-sided weakness or numbness, trouble speaking, fainting, severe new headache, chest pain, or inability to walk. Seek urgent assessment for sudden hearing loss, repeated vomiting, fever with severe dizziness, or symptoms after a fall or head injury. Arrange a clinical review if episodes disrupt sleep, recur with the same movement, cause falls, or continue during the day. Do not dismiss persistent symptoms as a dream or normal sleep transition.
