What should you know about vertical forehead line?

Short answer

A vertical forehead line is usually a facial crease related to expression, skin ageing, or changes in skin support; its appearance and treatment options depend on whether it is visible at rest or mainly during movement.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What a vertical forehead line can mean

Forehead lines are often linked to repeated muscle activity, such as raising the eyebrows or concentrating. Over time, a crease that first appears only during expression may remain noticeable when the face is relaxed. Sun exposure, dryness, smoking, and the normal reduction of collagen and elastic support can make the fold more visible. A single line does not by itself identify a medical condition.

The direction of a line can be hard to judge from a mirror or photograph, especially when nearby expression lines overlap. A clinician can examine the skin at rest and during movement, check whether there is irritation or a lump beneath it, and distinguish a surface crease from a different skin change.

What you can do

Use broad-spectrum sunscreen on exposed skin, moisturise regularly, avoid tanning, and do not smoke. These measures support the skin and may reduce additional damage, although an established crease may not disappear. Gentle cleansing is preferable to scrubbing or repeatedly stretching the area.

If the line bothers you, a dermatologist or suitably trained aesthetic clinician can discuss options. Botulinum toxin injection may soften lines driven by muscle movement, while other approaches may be considered when texture, pigmentation, or loss of volume is also present. Treatment should follow an examination, a discussion of expected change, and a review of possible side effects and aftercare.

When to seek care

Arrange a skin assessment if the mark is new and changing quickly, bleeds, crusts, repeatedly itches, becomes painful, or develops a persistent colour change. Seek prompt medical advice for swelling, weakness, or a new change affecting vision or movement around the eye. These features need assessment because they are not typical of an ordinary expression crease.

For a stable line with no other symptoms, care is generally elective. A clinician can still help if you are unsure whether you are seeing a wrinkle, a rash, or another skin lesion.

Questions for your doctor

Ask what is causing the line, whether it is present at rest or mainly with movement, which options fit your skin and medical history, how long improvement may last, and what aftercare or warning signs to expect.

Questions people ask

Is a vertical forehead line a sign of illness?

Usually it is a cosmetic skin crease, but a new, changing, bleeding, crusted, painful, or persistently itchy mark should be examined.

Can Botox help a vertical forehead line?

Botulinum toxin injection may soften a line that is strongly related to muscle movement. A qualified clinician must assess the pattern and discuss suitability, limitations, and risks.

How can I stop the line becoming more noticeable?

Use sunscreen and moisturiser, avoid tanning and smoking, and handle the skin gently. These steps support skin health but may not remove an established crease.
