VBAC delivery: planning, monitoring and alternatives

Your clinician will consider why the earlier caesarean occurred, the type of uterine incision, any other uterine surgery, the baby’s position and the placenta. The planned route of birth is also shaped by your health and the hospital’s ability to provide continuous assessment and urgent surgery. A vaginal plan can include a clear alternative if labour does not progress or concerns arise.

Share your operative notes and antenatal history with the team. Ask when to come to hospital, how contractions and fetal wellbeing will be assessed, and which pain-relief options are available. Discuss whether labour should begin naturally or whether induction is being considered. Write down the agreed plan and the circumstances that would change it.

Call your maternity service if contractions start, your waters release, bleeding appears or fetal movement decreases. Emergency evaluation is needed for severe continuing abdominal pain, heavy bleeding, fainting or sudden deterioration during labour. A prompt assessment helps the team distinguish expected labour changes from signs needing action.

Ask how your previous incision affects delivery choices, whether the baby and placenta create any concerns, how quickly an urgent caesarean could be arranged, and how you will be involved if the recommended route changes.
