VBAC after placenta previa: what to know

Short answer
VBAC after a previous placenta previa depends on where the placenta is now, the uterine scar, and whether your maternity team can respond quickly to bleeding or other labour concerns.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Current placenta
- Its position in this pregnancy matters more than a past location.
- Bleeding
- Any pregnancy bleeding with a previa history needs prompt maternity advice.
- Imaging
- Ultrasound helps assess placental position and fetal growth when indicated.
How a past placenta previa fits in
Placenta previa means the placenta was low in the uterus and may have covered or approached the cervix. A previous diagnosis does not by itself show where the placenta will attach in a later pregnancy. Ultrasound is used to assess its current position and its relationship to the cervix and the caesarean scar.
The team will also review the uterine incision and the reason for the earlier caesarean. If the current placenta overlies the cervix or bleeding is a concern, vaginal birth may not be appropriate.
What to discuss during pregnancy
Attend the recommended ultrasound appointments and ask when placental position will be checked again. A Growth Scan may help assess fetal growth when your clinician recommends it, but it does not replace assessment of the cervix or scar. Keep your prior operative and scan records available.
Ask where you should go if bleeding begins and whether your planned birth setting has blood, anaesthetic and surgical support. The final plan may remain open until later pregnancy findings are clear.
Bleeding needs prompt assessment
During pregnancy, contact your maternity unit urgently for any vaginal bleeding, even if it is light or painless, and seek emergency help for heavy bleeding, dizziness, faintness or severe pain. In labour, report bleeding, continuous abdominal pain, contractions or reduced fetal movement without delay.
Questions for your appointment
Where is the placenta in this pregnancy, and does it cover or approach the cervix? Will repeat imaging be needed? What did my previous caesarean report say about the uterine incision? Which symptoms require immediate hospital assessment?

Questions people ask
Does previous placenta previa rule out VBAC?
Not automatically. The current placental position, uterine scar and the rest of the pregnancy determine whether vaginal birth can be considered.
Can placenta previa happen again?
It can recur, so your maternity team will check placental location during this pregnancy and explain what the findings mean for birth.
