Why symptoms may cluster around bleeding
Changes around menstruation can affect the pelvic organs and bowel, making the bladder feel full or sensitive. Cramps, bloating, constipation, or diarrhoea may add pressure or discomfort. Urgency with burning, cloudy or strong-smelling urine, lower abdominal pain, or fever can fit a urinary tract infection and should not be dismissed as a period symptom. If urgency follows the cycle repeatedly, a clinician can consider bladder sensitivity, pelvic pain conditions, bowel factors, and other causes.

Note the first day of bleeding, urgency, drinks, pain, bowel changes, leakage, and whether symptoms disappear when the period ends. Drink regularly through the day and avoid large amounts of caffeine if they worsen urgency. Treat constipation with advice from a pharmacist or clinician, and use only pain relief that is suitable for you. Do not use leftover antibiotics or assume recurrent symptoms have the same cause each cycle.
When to seek care
Seek prompt assessment for fever, chills, back or side pain, vomiting, visible blood in urine, severe pelvic pain, or difficulty passing urine. Make an appointment if urgency lasts beyond the period, repeatedly interrupts daily activities, causes leakage, or comes with unusual bleeding or pain during sex. Testing may be useful even when symptoms appear cycle-related.
