How a strain behaves

The quadriceps at the front of the thigh, hamstrings at the back, and adductors along the inner thigh can all be strained. A strain often starts during sprinting, jumping, kicking, lifting, or a sudden change of direction. Pain may be sharp at the moment of injury, followed by tenderness, weakness, swelling, or bruising. Pain with a resisted contraction or a stretch can help locate the involved muscle. A cramp or post-exercise soreness has a different pattern, but an examination is needed when the diagnosis is uncertain.

Early and gradual care

Stop the triggering activity and protect the leg from movements that cause sharp pain. Use gentle walking only if it is comfortable; avoid sprinting, jumping, heavy resistance, and forceful stretching early on. A cold pack wrapped in cloth may reduce discomfort after the injury. When daily movement is easier, begin controlled range-of-motion and light strengthening, then add resistance, speed, and sport drills in stages. When medical assessment matters

Arrange assessment for a large bruise, a visible dip in the muscle, pronounced weakness, a limp, or pain that is not improving. Prompt care is important if you cannot bear weight or move the hip or knee normally. Seek urgent help after major trauma if swelling is rapidly increasing, the leg becomes numb or cold, or severe pain is out of proportion to the injury.
