What should you know about undulant fever?

The term describes a temperature pattern rather than a diagnosis. Fever may rise, ease, and return, with sweats, chills, headache, weakness, muscle or joint aches, and reduced appetite. Brucellosis is one possible cause, particularly after contact with infected animals or unpasteurised dairy, but many infections can produce changing fever patterns.

A clinician will ask when the fever began, how it behaves, recent travel, food exposures, animal contact, and medicines. Other explanations may include respiratory infections such as acute bronchitis or COVID-19, and fever with a skin problem may need assessment for cellulitis.

Arrange medical review if fever keeps returning, lasts beyond a short illness, or comes with drenching sweats, joint pain, unusual tiredness, or a relevant animal or food exposure. Record temperature readings, chills, sweats, medicines, travel, and what you have eaten or handled.

Rest, drink fluids, wear light layers, and use only fever medicine suitable for you according to its label or a clinician's advice. Do not take leftover antibiotics; testing can guide the right treatment.

Get urgent help for confusion, difficulty breathing, a stiff neck, a severe headache, fainting, persistent vomiting, or signs of dehydration such as very little urine. A rapidly spreading painful skin area also needs prompt assessment. Babies, older adults, pregnant people, and those with weakened immunity should seek advice earlier when fever is concerning.

Ask what could explain the fever pattern, whether blood or other tests are needed, whether brucellosis is relevant to your exposures, and which warning signs should change the urgency of care.
