Twisted knee without swelling: what should you know?

Short answer
A twisted knee can still have a meaningful ligament, meniscus, or muscle injury without visible swelling, so pain and function matter.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Visible swelling
- Its absence does not rule out an internal knee injury
- Useful clues
- Buckling, catching, locking, and pain with rotation
What may be happening
Swelling is only one possible response to a knee injury. A twist may irritate a ligament, the meniscus, or surrounding muscles while producing little fluid that you can see. Pain with turning, squatting, stairs, or straightening can provide more useful clues than appearance. A knee that catches, locks, buckles, or cannot fully move deserves attention even when its outline looks normal.
Other knee problems can cause pain or stiffness independently of a recent twist. Examples include a meniscus tear, knee bursitis, knee osteoarthritis, gout, or a Baker's cyst. These conditions do not all behave the same way, so the timing and exact location of symptoms help guide an examination.
Practical next steps
Reduce pivoting, kneeling, deep squats, and sport while the knee is painful. Use comfortable, easy movement rather than keeping it rigid all day, and support the leg during rest. A wrapped cold pack may calm soreness after activity. Do not test the knee repeatedly by twisting it, and do not return to sport until walking, bending, and controlled single-leg loading feel stable.
Record what caused the twist, whether the knee gave way, and which movements reproduce pain. This information helps a clinician distinguish a mechanical problem from irritation around the joint.
When to seek care
Seek urgent care after a major injury if you cannot bear weight, the knee is visibly deformed, it locks in one position, or the foot becomes numb or cold. Arrange an assessment for persistent pain, repeated buckling, a catching sensation, or reduced movement. New warmth, redness, or rapidly developing swelling also warrants prompt review.
Questions for your doctor
Ask which structures may have been strained, whether you need imaging, and which activities are safe now. Ask how to recognise locking or instability and whether joint aspiration has any role if swelling later appears.

Questions people ask
Can a knee injury hurt without swelling?
Yes. Irritated ligaments, meniscus tissue, or muscles may cause pain and movement problems without obvious fluid or puffiness.
When can I exercise again?
Resume gradually when ordinary walking and controlled bending are comfortable and the knee feels stable, rather than relying only on the absence of swelling.
