What should you know about twin VBAC?

VBAC means attempting vaginal birth after a caesarean. With twins, the team considers the position of each baby, the reason for the earlier caesarean, the type of uterine incision, your health, and how the pregnancy is progressing. The babies may change position, and labour can place extra demands on the uterus and maternity team. A planned birth setting should have staff, anaesthesia and surgical facilities available if an urgent caesarean becomes necessary. The birth plan may also include monitoring both babies and checking their heart rates throughout labour.

Discuss the option early with an obstetrician who can review your previous operative notes and current ultrasound findings. Ask how the babies are positioned, whether induction is suitable, and what signs would change the plan. Keep appointments so growth, presentation and placental position can be assessed. Contact your maternity unit urgently for vaginal bleeding, fluid loss, regular painful contractions, reduced movement of either baby, severe abdominal pain or feeling faint. Do not wait for a routine appointment when symptoms are sudden or intense. Your local maternity service can advise where to go and how quickly you need assessment.
