What tonsil-area pain can mean
Pain, redness, swelling, or white patches around the tonsils can occur with tonsillitis, strep throat, a common cold, influenza, COVID-19, or glandular fever. A blocked or runny nose can drip into the throat and irritate it, as can allergic rhinitis. Acid reflux may cause burning, throat clearing, or discomfort without a typical cold. The appearance of the tonsils alone cannot identify the cause; fever, cough, swollen neck glands, rash, fatigue, and exposure to illness add useful clues.
Most sore throats improve as the underlying irritation settles. Antibiotics help only selected bacterial infections, so a clinician may use an examination and, when appropriate, testing before recommending them.
What you can do now
Rest, drink fluids regularly, and choose soft foods if swallowing hurts. Warm drinks or cool foods may soothe the throat; avoid smoke and other irritants. Follow label directions for suitable pain relief, taking account of allergies, pregnancy, age, and other medicines. Do not share utensils, and wash hands if an infection is possible. If symptoms keep returning, note triggers such as reflux, nasal allergies, or contact with someone who is unwell.
When to seek care
Seek urgent help for trouble breathing, inability to swallow saliva, drooling, a muffled voice, severe one-sided swelling, difficulty opening the mouth, or rapidly worsening neck swelling. Arrange medical assessment for persistent or recurrent pain, significant fever, dehydration, a widespread rash, marked weakness, or symptoms after close contact with strep throat or another infection. A clinician can check the tonsils, hydration, and neck glands and decide whether testing or treatment is needed.
